Local district Special Education Directors
may submit a Form 2 requesting an evaluation,
consultation, and/or service to the MISD,
Support and Related Services Department.
Services for Occupational Therapy, Physical
Therapy, Vision and Orientation & Mobility
Services may be requested to support a
student's special education goals.
Referral materials must include:

Copy of the most recent IEPT
-
Referral Form (for Occupational and/or
Physical Therapy)

Form 2 Referral must be signed by both the Parent
and
the Special Ed Director

Current eye report (for Vision services)
