Fee: A "NO SHOW" fee will be charged of $90.00

Instructions:
Classes begin promptly at 7:00 a.m. and end at 1:00 p.m. with one half hour for lunch (please bring your own lunch) and one additional break.  When a Registration Form is received, via van mail, fax or email, a Course Enrollment Card (White Card) will be issued and is valid for ten (10) days after the completion date of the course, but not to exceed ninety (90) days.  A signed original Registration Form must be sent to the Business Office.  Drivers must attend all four (4) classes and pass the test to receive a Course Completion Card (Yellow Card).  Course Completion Cards (Yellow Card) will be mailed to the local school district Transportation Director within 10 days of course completion.  In case of a student cancellation, MISD must be notified 10 days prior to scheduled class date.  Any student changes must be made no less than three (3) business days from start of class.  A "NO SHOW" fee will be charged of $90.00.

Persons arriving late will not be admitted to class.

Fee: A "NO SHOW" fee will be charged of $25.00
Maximum Class Size:  40

Instructions:
Classes begin at 7:30 a.m. and end at 2:30 p.m. with a one hour lunch.  A Registration Form must be received, via van mail, fax or e-mail to enroll students.  A signed original Registration Form must be sent to the Business Office.  Please fill out a separate Registration Form for each class date.  In case of a student cancellation, MISD must be notified 10 days prior to scheduled class date.  Any student changes must be made no less than three (3) business days from start of class.
