Purpose/Function:
Center programs for the deaf/hard of hearing, located in the Lakeview, Utica, and Roseville school districts, offer classroom placement for students who are deaf/hard of hearing.
Oral programs, located in Lakeview and Utica, provide training in listening, speaking and speech reading. The Lakeview and Utica programs serve their surrounding area. Utica Community Schools provide oral programs from preschool through elementary school. The Lakeview School District provides oral programs for preschool, elementary, middle and high school students..
Total communication programs, located in Roseville and Lakeview provide training in listening, speaking, speech reading, signing and finger spelling. The Roseville total communication program serves the entire county with classrooms from preschool through junior high. The Lakeview total communication program provides the high school program (grades 09-12).
The need for a special classroom for students with Deaf/Hard of Hearing may be based on the following considerations:
Preschool Child:
- Degree of hearing loss
- Effect on language development
- Need for training in the use of hearing aids
- Need for intensive language development program
School-Age Child:
- Severe language delay
- Inability to successfully function in the general education
- Lack of understanding of the language
- Need for training in the use of hearing aids,
- Need for the development of attention skills for communication,

Referral Process:
A Form 2 referral is sent by the local district Special Education Director to the MISD Director of the Assessment Center and the operating Director (Lakeview, Utica, Roseville).
A Form 2 may be initiated for the services of an MISD teacher consultant for the deaf/hard of hearing (TC) to assist when a student is being considered for Deaf/Hard of Hearing classroom. Please check “Consideration for Program Placement, deaf/hard of hearing.” on the form.
Similarly, a request may be made for a TC for assistance with students in the general education setting of a local school district. Please check "Deaf/Hard of Hearing Consultation" on Form 2.
The Local Director should contact the Operating Director of the Center Program to arrange for parents to visit the Deaf/Hard of Hearing program.
If a referral is being made by a non-resident district, a copy of the referral Form 2 must be forwarded to the Director of the student's resident district.
*For School of Choice (105c out-of-county), a written cooperative agreement must be approved between the resident district, enrolling district, the resident ISD, and the Macomb ISD Assistant Superintendent of Special Education.
The following documents should be obtained by the local district and enclosed with the referral:
- Student Identified as deaf/hard of hearing or HI Transfer Student with disability.
- Current Eligibility Report(s)
- Current IEP
- Current REED Report(s)
- Current Audiological Evaluation
- FBA and or Behavior Intervention Plan (if Applicable)
- Birth Certificate
- Immunization Record
- Current Otology/ENT Evaluation (Medical)
- Speech and Language Evaluation
- Teacher Progress Report
- Psychological
Referral Response:
There is a review of the referral package and contact with the local district for clarification or additional information, if needed.
MISD completes a Recommended Disposition Form 2A and sends a copy to the operating district (with the referral package) and to the resident district.
The sending district is responsible for all arrangements for the IEP.
- The sending district is responsible for scheduling and inviting all required participants to the IEP.
- The sending district TieNet Liaison will provide access to the HI Center Program for the purpose of accessing TieNet Documents.
- The sending district initiates the IEP (PLAAFP, Summary, Services, HI Center Programs will write goals).
- The sending district chairs the meeting in cooperation with the receiving school administrator/designee.
Outcome:
The resident district contacts the receiving district and the MISD Assessment Center Director and HI Teacher Consultant (if applicable) to schedule an IEP meeting.
NOTE 1: Resident District:
- Request to the MISD Transportation Department for service.
- Finalizes the IEP in TieNet.
NOTE 2: This referral process and outcome is followed even if the student being considered resides within the district operating the HI center program.
Exceptions:
Students Previously Enrolled in another District
Students transferring to the local district from outside the county who are being served in a similar program may be referred directly to the MISD for LEA Center placement.
For immediate placement:
- The Referring district must complete transfer of student with a disability and submit it with Form 2 to the MISD and the Operating HI Center district.
- The Referring district must also request and collect data from the Previous district and submit this to MISD and the Operating HI Center district within 30 days.
- The Operating HI Center district must hold a new IEP within 30 school days.
If the student is transferring from one center program (e.g., Oral) for the deaf/hard of hearing to another (e.g. Total) within the county, a Form 2 Referral is sent to the MISD Director of Assessment Center by the current resident district and the appropriate Center Program Director. Include with the Form 2 referral:
- Current IEP, MET and REED/Evaluation Review report
- Most recent audiological and otological reports.
- Other evaluations as outlined under REFERRAL PROCESS should be included with the referral.
Visitation Policy:
Director of Special Education/Supervisor contacts the LEA or ISD Center Program Director to set up visitation.
