Placement in special classes, separate schooling, or other removal of a student from the regular education environment occurs only when the nature or severity of the disability is such that education in a regular class with the use of supplementary aids and services cannot be achieved satisfactorily.

Purpose/Function

To provide service to students ages 3 to 18 years of age with moderate cognitive impairments. There are four programs, two of which are in the LEA MoCI Center Program Consortium and two of which are located in MISD MoCI Center Programs. The curriculum is based on the educational and functional needs of children.

Note: LEA MoCI Center Programs provide students the opportunities to participate with non-disabled peers. MISD MoCI Center Programs provide a restrictive environment, and more support with students exhibiting significant medical challenges.

Referral Process

1. Form 2 signed by Director of Special Education and parent , is submitted to Director of the Program's catchment area. Placement consideration is based on the catchment areas below.

- If the referral is for a MISD MoCI Center Program, a referral packet should be submitted to the Director of Center Programs at the MISD Educational Service Center, 44001 Garfield Road, Clinton Township, MI 48038, (586) 228-3431.
- If the referral is for a LEA MoCI Center Program, a referral packet should be submitted to the Director of that catchment area (i.e. Warren Consolidated Special Education, 31300 Anita, Warren, MI 48093 (586) 698-2129 or Lake Shore Special Education 28850 Harper, St. Clair Shores, MI 48081, (586) 285-8610).

2.  If a referral is being made by a non-resident district, a copy of the referral Form 2 must be forwarded to the Director of the student's resident district.

3.  For School of Choice (105c out-of-county), a written cooperative agreement must be approved between the resident district, enrolling district, the resident ISD, and the Macomb ISD Assistant Superintendent of Special Education.

4.  Referral materials must include:

• Current Eligibility Report(s) with supporting reports (i.e. Psychological reflecting scores by age not grade, standardized adaptive scale, Speech & Language Report).
• Current IEP
• Current REED Report(s)
• Summary of reason for referral (i.e. behavioral concern, etc.)
• Behavior Intervention Plan
• Birth Certificate
• Immunization Record
• Medical Information
• Current Teacher Input Form

Referral Response:

1. After receiving the referral materials, the Director of the appropriate Center Program will review the packet. The following may occur:
   - Contact LEA Special Education Director and may arrange an observation of the student in their current placement.
   - Packet incomplete, request additional information from referring district.
     
2. If referral to Moderate Cognitive Impairment Program is considered appropriate:
   - A Form 2A - Report of Recommended Disposition will be sent to District.
   - A copy of Form 2A will be sent to:
     a). The appropriate administrator (Director of Special Education of operating district or Principal of MISD Program) and
     b). The Director of Special Education of the referring school district recommending that an IEPT be scheduled to consider placement.
      
3. The sending district is responsible for all arrangements for the IEPT.
   - The sending district is responsible for scheduling and inviting all required participants to the IEP meeting.
   - The sending District TieNet Liaison will provide access to the MoCI Center Program TieNet Liaison for the purpose of accessing TieNet Documents.
   - The sending district will create and complete the following sections of the IEP: Cover Page, Student Summary, and PLAAFP. MoCI Center Programs will collaborate on goals and objectives and other sections.
   - The sending district chairs the meeting in cooperation with the receiving school administrator/designee.

Outcomes:

An IEP is held to consider center program placement.

Visitations Policy:

MISD MoCI Principal or LEA MoCI Director/Supervisor contacts the referring Director of Special Education/Supervisor after submission of Form 2. For other arrangements, please contact the Director/Supervisor of LEA MoCI Program or Principal of MISD MoCI Program.

Exceptions:

Expedited Form 2 Placement Process- this is for students coming out of a long-term, very restricted, separate facility, who clearly needs the most restrictive educational setting.

The LEA District must:

1. Have parent establish proof of residency within their district
2. Have parent register with resident district
3. Immediately contact the Director of Center Programs to discuss the student and previous programs and services.
4. Complete Form 2 with required signatures
5. Attach most current IEP, MET/REED and/or supporting documentation indicating educational need for most restrictive placement
6. Deliver completed hard copy packet to Center Programs
7. LEA Director/Supervisor will be directed to the appropriate Center Program Administrator.

Out-of-State Students will require a REED and the district should conduct a MET and/or review characteristics for eligibility by State of Michigan requirements to ensure that the student meets Michigan eligibility requirements. The resident district may request the assistance of the appropriate Director of the LEA/MISD Center Program.

International Students moving into the county suspected of having severe cognitive, physical and or behavioral disability, the resident district should immediately conduct a full and comprehensive evaluation. The resident district may request the assistance of the appropriate Director of the LEA/MISD Center Program.

Timeline:

Receipt of the signed and completed Form 2 Referral may be completed within 30 school days. Situations occur that may delay the referral process, such as, incomplete referral packets, students in the juvenile justice system, hospitalization, out of date evaluations, etc.

For fall placement referrals must be received between April 1st and May 1st. This will allow for the Centers to arrange IEPs and Transportation.
