Program for Students with Physical Impairment/Other Health Impairments

Placement in special classes, separate schooling, or other removal of a student from the regular education environment occurs only when the nature or severity of the disability is such that education in a regular class with the use of supplementary aids and services cannot be achieved satisfactorily.

Purpose/Function:

LEA Center Programs for students with Physical Impairment/Other Health Impairments offer classroom placement for pre-primary aged students through high school age (including up to post-secondary) who manifest a physical or health impairment that adversely affects the student’s academic performance and may require physical adaptations within the school environment.

The determination of the need for the special education PI/OHI classroom varies with each student. These classrooms are designed for students with an IEP with average to mild cognitive abilities who require intensive physical and/or medical needs that can not be provided with reasonable supports in the local district (i.e. nursing care).

Referral Process:

A Form 2 Referral is sent directly to the appropriate LEA Director operating the PI/OHI program within the resident school district’s catchment area.

For School of Choice (105c out-of-county), a written cooperative agreement must be approved between the resident district, enrolling district, the resident ISD, and the Macomb ISD Assistant Superintendent of Special Education.

If a referral is being made by another Center Program (MISD/LEA), a copy of the referral Form 2 must be forwarded to the Director of the student's resident district.

The following documents should be enclosed with the referral:

  • Current Eligibility Report(s) including medical/therapy reports
  • Current IEP MUST include present levels in all domains
  • Current REED Report(s)
  • Psychological which shall include a Cognitive and Adaptive Assessment
  • Birth Certificate
  • Immunization Record
  • Referring Teacher Input Form
  • Functional Behavior Assessment/Behavior Intervention Plan (If Currently Implemented)

Referral Response:

Upon receipt of the signed Form 2 Referral the following procedures should be completed within 30 school days:

  1. The PI/OHI LEA Director reviews student’s records.
  2. The LEA Center may set up an observation of the student in their current placement.
  3. The PI/OHI LEA Director sends Form 2A Disposition to sending District.
  4. The MISD PI/OHI Consultant is available to assist the PI/OHI Directors as requested.

Visitation Policy:

PI/OHI LEA Director/Supervisor contacts the referring Director of Special Education/Supervisor after submission of Form 2. For other arrangements, please contact the Director of LEA PI/OHI Program.

Outcome:

The resident district contacts PI/OHI LEA Director to schedule the IEPT meeting a mutually convenient time and place with the parent.

The sending district is responsible for all arrangements for the IEP.

  • Scheduling and inviting all required participants to the IEP.
  • District TieNet Liaison will provide access to the PI/OHI LEA Center Program for the purpose of accessing TieNet Documents.
  • Prepare draft IEP sections in TieNet. PI/OHI LEA Center Program will collaborate on writing goals and objectives.
  • Chairing the meeting in cooperation with the receiving school administrator/designee.

Required members of IEP Team at a minimum consist of:

  • The sending school district
  • The receiving school district
  • The parent (or the students, if appropriate)
  • The resident district, if not the sending or receiving district.

Timeline:

Upon receipt of the signed and completed Form 2 Referral the procedures may be completed within 30 school days. Situations occur that may delay the referral process, such as, incomplete referral packets, students in the juvenile justice system, hospitalization, out of date evaluations, etc.

Exceptions

Transfer of Student with a Disability

Students transferring to the local district from outside the county who are being served in a similar program may be referred directly to the MISD or LEA Center. In addition, students who are returning from a hospital or other specialized setting may be referred at the discretion of the LEA Director.

  • Must verify proof of residency with resident district
  • Must include copy of birth certificate and immunization record
  • Request release of records from previous placement/district

In such cases, for immediate placement, the Referring District must complete a Transfer of Student with Disability Form and submit it with the Form 2 to the MISD or LEA Center. Also, the referring district must request and collect data from the previous district and submit this to MISD or LEA Center within 30 days.

If the student is transferring from one elementary PI/OHI program to another within the county the Form 2 Referral is sent between special education directors. The records should include current MET report, IEPT report, temporary placement form, if necessary and other evaluations as outlined under the Referral Process and be included in the Referral.

When a PI/OHI elementary program has reached the maximum number of students according to the State guidelines, the special education director of the resident district will contact the next closest elementary PI/OHI program special education director to determine the feasibility of placement in that program.

PI/OHI LEA Center Program Director contact the MISD Assistant Superintendent of Special Education & Student Services regarding Out of catchment placement.
