Transition planning for Early On or MIPP children begins between 27 and 33 months of age. This process may involve meetings, visits, and team decision-making. Parents are important members of the transition team. The service coordinator will also be a part of the transition process, as well as a representative of the local school district, if appropriate.
Parent Guides
